At least 77 people were injured during a train incident in the Costa Rican capital of San Jose, according to local media.

MOSCOW (Sputnik) – A head-on collision of two commuter trains in the Costa Rican capital of San Jose left at least 77 people injured, national media reported Friday.

Five patients, including two engineers and two assistants, have been transferred to medical centers in serious condition, Costa Rica’s La Nacion daily reported. Forty-one people came away with minor injuries, and 31 were in stable condition.

"I do not understand why this lack of communication. Knowing that there is an interchange line ahead, I do not understand why there was no communication," one of the passengers en route to work told the newspaper.

The accident reportedly took place on a track curve in the Pavas district of San Jose some 1,600 feet from the Ministry of Agriculture and Livestock building.

Rail transport authority Incofer President Guillermo Santana was quoted as saying in a briefing that preliminary findings pointed to human error.

"Lack of radio communication can be one of the causes," Santana said.

The Red Cross, fire department, traffic police and national security forces assisted in the rescue effort.

"Investigators and Attorney suspect that to rail accident in February this year in Germany came as the switchman played games on the phone.

Investigators found out that Michael P. during his service on the day of the accident on February 9 turned on his cell phone and played online games. Played it is active even a few moments before the horrible crash. It is suspected that he is distracted from the proper performance of the job. It is suspected that it is therefore made the wrong assumptions and trains gave the wrong signals and pressed the wrong button which is why the driver did not hear warning."

3 dead, 40 injured as passenger, freight trains crash in Belgium
http://presstv.com/Detail/2016/06/06/469121/-Belgium-train-crash-dead

“A train... carrying around 40 passengers crashed into the back of a freight train on the same track,” Belgian railways SNCB said in a statement late on Sunday.

The collision, which took place in the eastern municipality of Saint-Georges-Sur-Meuse, derailed two of the six carriages of the passenger train.

Local emergency services and reinforcements from the nearest city of Liege rushed to the site of the crash. A crisis center was set up at the scene.

Françis Dejon, the mayor of the Saint-Georges-Sur-Meuse district told Belga news agency that the impact of the crash was “very violent.”

Witnesses said the passenger train was travelling at high speed when it crashed turning the front carriage on its side.

Several passengers had to be extracted from the wreckage.

News on the Colorado Northern June 12th, 2016:

A 59 car freight derailed today near Golden. It was being hauled by 3 leased locomotives from UP (two SD70Ms and 1 ES44AC) on the front, and 1656 and 675 on the back pushing. They hit a broken rail along one of the straight sections of track, however, falling into the mud and derailing. The snow is actually the fake type made out of paper meant for a movie being filmed, and some of it got on the locomotives due to the wind and derailment. The front ES44AC and SD70M were heavily damaged in mainly their generators and fuel tanks, and have been moved to Golden to await their final end as scrap. The other SD70M has been moved to a Golden scrapyard, but mainly for storage in case it isn't needed to be scrapped. 5 people died, 3 of them being crews on both the front locomotives, and 2 who were part of the movie being filmed as a test to see their performances, who were killed by the locomotive. Their bodies had been removed at the time of this picture's taking.

Five people, including three children, have been killed in the US State of Colorado after their van collided with an Amtrak train, police say.

Train crash kills 5 people in Colorado

The incident happened on Sunday morning, when a Chrysler vehicle was struck by the train near Trinidad, the Colorado State Patrol said in a statement.

Investigators believe failing to yield to the train is the main factor behind the five deaths.

The van’s driver and three of its six passengers were pronounced dead at the scene of the crash while the fifth passenger died at a hospital.

Colorado police also said that a 4-year-old girl who had sustained serious injuries was flown to a nearby children’s hospital as the only survivor.

The identities of the passengers were not immediately released as police were still investigating the cause of the accident and had to notify the families of the victims first.

According to an Amtrak spokesman, more than 280 passengers were on board the train at the time of the crash but no one was injured.

The Southwest Chief train, which travels between Chicago and Los Angeles, was damaged and crews had to make repairs before it could travel again at around 5 p.m.

Christina and Steve Miller were driving their four young girls to church when they collided with train

5 family members killed in Amtrak crash near Trinidad were going to church, cousin says (Video)
http://www.denverpost.com/2016/06/27/colorado-amtrak-train-crash-family-killed/

The five people, including three young children, killed when their minivan was struck Sunday afternoon by an Amtrak train in southern Colorado were a family heading to church, according to the cousin of one of the victims.

Keith Schlabach, the cousin of Christina Miller, said she was with her husband, Steve, and their four girls when they died in the collision about 3 miles north of Trinidad.

“We were at church and when they didn’t show up for church, we were worried about what was going on,” Schlabach said Monday morning. “We thought they were broken down or something. One of the other guys was driving back from church toward Steve’s house to see what happened, and then he got to the tracks.”

Schlabach said the route the Millers normally took to Trinidad Christian Church, an unincorporated congregation, crossed the train tracks.

The Colorado State Patrol on Monday formally identified the Millers as the victims in the crash. Stephen Miller, 32, was behind the wheel of the family’s 2005 Chrysler Town & Country minivan with his wife, 33-year-old Christine Miller, in the passenger seat.

Their daughters, a 6-year-old, a 2-year-old and an 8-month-old, died in the crash, according to Schlabach and authorities. A GoFundMe page created to fund the family members’ funeral identified the daughters’ names, respectively, as Abigail, Kathryn and Ellianna.

A fourth daughter, 4-year-old Heidi, was seriously hurt in the collision and transported by helicopter to Children’s Hospital Colorado. Family said she was stable Monday morning.

Investigators say the minivan was northbound on Las Animas County Road 32 when it failed to yield right of way to the Amtrak train — a Los Angeles-bound Southwest Chief — which was traveling westbound. The front of the train collided with the right side of the Chrysler.

The crossing does not have gates or warning lights and is marked by just four crossbuck signs, according to authorities.

After impact, the Chrysler was pushed westbound on the railroad tracks till the train came to a stop, authorities say. The passengers and crew members aboard the train were not injured.

“Steve was very likable,” Schlabach said. “He was very outgoing. Everybody got along with Steve. He’s just that kind of guy. He really liked reaching out to people — reaching out to people and making friends.”

The Southwest Chief has been involved in at least one other high-profile incident with a vehicle in recent months.

Investigators blamed a March derailment of a Southwest Chief train in Kansas that injured more than two dozen people on damage to track that was traced to a grain truck. The Associated Press reported a National Transportation Safety Board member said the truck’s impact shifted the train tracks 12 to 14 inches.

On May 31, the driver of a pickup truck was cited for failing to obey a railroad crossing after investigators say he drove into the path of Amtrak’s California Zephyr, the rail carrier’s other route through Colorado, in Glenwood Springs.

The driver, 40-year-old Ever Vega Vega, had only minor injuries after that crash.

Vehicle Accident in USA on June 28 2016 04:11 PM (UTC).

Situation Update No. 1 on June 28 2016 06:33 PM (UTC)
Two freight trains have collided near the city of Panhandle, Texas. The wreckage is on fire, and a portion of the city was evacuated due to the heavy smoke. One person was injured, while rescue crews are looking for 3 more. The collision took place Tuesday morning, about five miles (8 km) east of Panhandle, alongside US Highway 60 towards Amarillo. Both trains were identified as belonging to BNSF, one of the largest freight railroads in North America and a subsidiary of Warren Buffett's Berkshire Hathaway Inc. Little is known about the cause of the crash at this time. Unconfirmed reports say the collision was head-on. Panhandle authorities have asked residents to reduce their water usage by turning off sprinklers, because the city's water supply is being depleted by firefighters at the collision site, KVII reported. The east side of the city has been ordered to evacuate, because the shifting winds started blowing smoke towards the residential areas. It is unknown what cargo the trains were carrying, however. Carson County officials are asking drivers to avoid the area, so the emergency crews can focus on rescue work. Police have complained that passing motorists are leaving their cars to take pictures of the accident, putting more people in danger, KCBD reported. There were four employees on board the trains, BNSF spokesman Joe Faust confirmed. At least one person was injured, according to KFDA, and is reportedly in stable condition at Northwest Texas Hospital. Rescue crews are looking for the remaining three.

Taiwan’s Central News Agency cited police as saying they suspected the tonight blast in the capital of Taipei had been caused by some kind of explosive in the train carriage.

The news agency cited the director of Taiwan’s National Police Agency, Chen Kuo-en, as saying late today that the explosive appeared to be 15 to 20 centimeters (6-8 inches) long and looked like a firecracker.

The report says the self-ruled island’s premier, Lin Chuan, ordered government agencies to form a team to investigate the blast.

The report cites unidentified witnesses as saying they heard a loud explosion before the train carriage burst into flames.

Taiwan has had its share of explosions, once in 2014 and the other in 2015. In 2014, a series of explosions occurred in Kaohsiung district following reports of gas leaks the night before. At least 32 people were killed and around 321 others were injured.

In 2015, a fire on a music stage spread into a crowd of spectators at a Formosa water park in Taiwan where more than 500 people were injured, fifteen of them died. The fire was sparked by an accidental explosion of an unknown powder near the stage in front of about 1,000 people.
